If I were to compare a shounen character that’s similar to Tsuna, personally I’d say Ichigo Kurosaki from Bleach. Mafias or Yakuzas are and were true to our world to this very day, something that most of us tend to be afraid of due to their violent history and acts,similarly to how Tsuna fears them. Tsunayoshi Sawada on the other hand is much more relatable, because he represents realistic personality even still all the way to the end of the series, especially by the fact that he lives in a realistic world with no superheroes, no shinobis and no pirates, but the world that Tsuna deals with are mafias, and interestingly enough. Now, Midoriya lives in a world that’s filled with super heroes with special powers called “quirks”, it’s also important to note that Midoriya himself is and was aware of the bizarre things around him such as witnessing people having these special powers/abilities that happens to exist in the world that he lives in, and because of that, he suddenly has the urge of hoping to gain these special powers of his own to be just like All Might, the hero that he admired as a child, and is willing fit in with other people with special powers, the more story progressed, the more confident he became and the less fear he developed, because the hero of his childhood later on finally acknowledged him and gave him these special powers to become the new hero and protect the world. Now you can argue that Tsuna and Midoriya are quite similar due to their personality, but the world and the situation that they are in is completely backwards. Personally, I have a reason to believe that Tsuna’s anxiousness is what makes him relatable as a character, any middle school student being thrown in a situation like Tsuna’s would crack from pressure and lose themselves over the horrors of the world that Tsuna deals with.
